Abstract Personalized mRNA therapeutics represent an emerging frontier in precision medicine, combining messenger RNA technology with genomics, artificial intelligence, and individualized clinical decision-making. Unlike conventional pharmaceutical approaches, mRNA platforms can be rapidly designed to encode therapeutic proteins, antigens, or other biologically relevant molecules, creating opportunities for patient-specific interventions. Advances in genomic sequencing, computational biology, machine learning, and lipid nanoparticle delivery are increasingly enabling the identification of molecular targets and the development of individualized mRNA constructs. This paper examines the technological foundations of personalized mRNA therapeutics and evaluates their potential applications in cancer immunotherapy, rare diseases, infectious diseases, protein replacement, and regenerative medicine. A qualitative and conceptual methodology is adopted to analyse the integration of genomics, artificial intelligence, mRNA design, delivery systems, and clinical decision-making. The analysis indicates that AI can support target identification, sequence optimisation, neoantigen prediction, patient stratification, and treatment-response modelling, while genomic information provides the molecular basis for individualised therapeutic design. Major challenges include delivery efficiency, RNA stability, immunogenicity, manufacturing scalability, computational bias, genomic privacy, regulatory complexity, and equitable access. The study proposes an integrated precision-mRNA framework connecting genomic profiling, AI-assisted target selection, computational sequence design, personalised manufacturing, and clinical monitoring. The paper concludes that personalized mRNA therapeutics could significantly expand the scope of precision healthcare, provided that technological innovation is accompanied by rigorous clinical validation, secure genomic-data governance, scalable manufacturing, and responsible regulatory frameworks.
